OFFICIAL USDA CERTIFICATION:
Some countries require that a veterinarian certificate issued in
the United States have an official USDA stamp. The following is
the procedure for acquiring one.
Have your veterinarian complete the EU form 998 or other form
for the country you are visiting. Your veterinarian will
complete and sign the form including the vaccination information
and stating that the pet is parasite free and in good health.
Take the original of the form to the APHIS/USDA vet office in
your state and they will endorse and stamp them. There is a fee
per form...more than 1 animal can be listed on a form. If your
vet uses separate forms for each animal; there will be a separate fee per
form. If there is not a State office located nearby you can send
the forms by mail.
Include: The original of the form
The payment in the form of a check or money order.
A self-addressed, stamped envelope for returning the form to you.
